Welcome to the July 2014 homepage edition of i2P (Information to Pharmacists) E-Magazine.
At the commencement of 2014 i2P focused on the need for the entire profession of pharmacy and its associated industry supports to undergo a renewal and regeneration.
We are now half-way through this year and it is quite apparent that pharmacy leaders do not yet have a cohesive and clear sense of direction.
Maybe the new initiative by Woolworths to deliver clinical service through young pharmacists and nurses may sharpen their focus.
If not, community pharmacy can look forward to losing a substantial and profitable market share of the clinical services market.
Who would you blame when that happens?
But I have to admit there is some effort, even though the results are but meagre.
In this edition of i2P we focus on the need for research about community pharmacy, the lack of activity from practicing pharmacists and when some research is delivered, a disconnect appears in its interpretation and implementation.

Woolworths Pharmacy - Getting One Stage Closer

It started with “tablet” computers deployed on shelves inside the retailer Coles, specifically to provide information to consumers relating to pain management and the sale of strong analgesics.
This development was reported in i2P under the title Coles Pharmacy Expansion and the Arid PGA Landscape”
In that article we reported that qualified information was a missing link that had come out of Coles market research as the reason to why it had not succeeded in dominating the pain market.
Of course, Woolworths was working on the same problem at the same time and had come up with a better solution - real people with good information.

Community Pharmacy Research - Are You Involved?

Government funding is always scarce and restricted.
If you are ever going to be a recipient of government funds you will need to fortify any application with evidence.
From a government perspective, this minimises risk.
I must admit that while I see evidence of research projects being managed by the PGA, I rarely see community pharmacists individually and actively engaged in the type of research that would further their own aims and objectives (and survival).

Organisational Amnesia and the Lack of a Curator Inhibits Cultural Progress

Hmm. There’s more than one way you could take that, huh? Like Someday when I get around to it (I’m not sure) I may admit that I was wrong about something. No juicy confessions this month except that I wish it were easier for me to admit when I have been wrong or made a mistake.

Dispensing errors – a ripple effect of damage

Most readers will be aware of recent publicity relating to dispensing errors and in particular to deaths caused by methotrexate being incorrectly packed in dose administration aids.
The Pharmacy Board of Australia (PBA), in its Communique of 13 June 2014, described a methotrexate packing error leading to the death of a patient and noted “extra vigilance is required to be exercised by pharmacists with these drugs”.

Explainer: what is peer review?

We’ve all heard the phrase “peer review” as giving credence to research and scholarly papers, but what does it actually mean?
How does it work?
Peer review is one of the gold standards of science. It’s a process where scientists (“peers”) evaluate the quality of other scientists' work. By doing this, they aim to ensure the work is rigorous, coherent, uses past research and adds to what we already knew.
Most scientific journals, conferences and grant applications have some sort of peer review system. In most cases it is “double blind” peer review. This means evaluators do not know the author(s), and the author(s) do not know the identity of the evaluators.
The intention behind this system is to ensure evaluation is not biased.
The more prestigious the journal, conference, or grant, the more demanding will be the review process, and the more likely the rejection. This prestige is why these papers tend to be more read and more cited.

Evidence-based medicine is broken. Why we need data and technology to fix it

The following article is reprinted from The Conversation and forms up part of our library collection on evidence-based medicines.
At i2P we also believe that the current model of evidence is so fractured it will never be able to be repaired.
All that can happen is that health professionals should independently test and verify through their own investigations what evidence exists to prescribe a medicine of any potency.
Health professionals that have patients (such as pharmacists) are ideally placed to observe and record the efficacy for medicines.
All else should confine their criticisms to their evidence of the actual evidence published.
If there are holes in it then share that evidence with the rest of the world.
Otherwise, do not be in such a hurry to criticise professions that have good experience and judgement to make a good choice on behalf of their patients, simply because good evidence has not caught up with reality.

Culture Drive & Pharmacy Renewal

Deep within all of us we have a core set of values and beliefs that create the standards of behaviour that we align with when we set a particular direction in life.
Directions may change many times over a lifetime, but with life experiences and maturity values may increase in number or gain greater depth.
All of this is embraced under one word – “culture”.
When a business is born it will only develop if it has a sound culture, and the values that comprise that culture are initially inherent in the business founder.
A sound business culture equates to a successful business and that success is often expressed in the term “goodwill” which can be eventually translated to a dollar value.

Neil Johnston is a pharmacist who trained as a management consultant. He was the first consultant to service the pharmacy profession and commenced practice as a full time consultant in 1972, specialising in community pharmacy management, pharmacy systems, preventive medicine and the marketing of professional services. I was thumbing through my January copy of the AJP when I noticed a small column covering a conversation with Nicola Roxon, the ex-Minister for Health and Ageing.
She, along with other commentators on the same page, was basically encouraging pharmacists to “jump in” to reform health.
The encouraged pathway was through fee for service arrangements, some of which are covered under the 5CPA.

This is a theme that i2P has been promoting for nearly a decade with mounting intensity towards the end of 2012, because it is obvious that unless additional income streams become available to pharmacists, employment will continue to suffer suffer, salaries will decline and pharmacy proprietors will progressively go out of business.

I think most pharmacists are fast coming to this realisation and are also anticipating some sort of crisis past April 1st 2012 when a range of high volume generic drugs will be slashed in price.
PBS affordability and sustainability are the reasons behind all these “reforms” – a word that I intensely dislike, because in government terminology this means slashing prices on the PBS without a great deal of thought to the people trying to sustain pharmacy practice at the coalface.

No other form of business I am aware of has to put up with constant change in regulations resulting in downward spirals of income and profit. In dealing with government this becomes a cycle of perpetual motion.
Generally, it can be said that the reform cycle is a result of government mismanagement and the lack of understanding of simple business principles.
Certainly, a major misunderstanding of the impacts that small businesses have to endure at the hands of a multitude of government agencies, and their bureaucratic regulations.

Reform offsets in the form of the funding of a range of “top-down” services made available through the infrastructure support of the Pharmacy Guild of Australia (funded by government) will not be the answer, because the uptake of each service requires a commitment to time and monetary contributions from each pharmacist within a pharmacy.
They have to be trained and often some important, but often forgotten personnel are left out. For example, locum and part-time pharmacists are often unaware that new systems are in place and no briefing has occurred to bring them up to speed.

So while Nicola Roxon implies a “sanction” for pharmacists to jump in and develop clinical services, the individual pharmacist is given no direct financial incentive to bring their skill set up to date, or develop from scratch a workable “bottom-up” type of clinical service.
Unless an entrepreneurial pharmacist proprietor is prepared to accept the entire financial risk and go against the tide to develop clinical services that would actually work.
If such a person did exist and wished to genuinely form an independent alliance with a pharmacist contractor then a new business model for pharmacy would stand a chance for survival, with costs shared equitably between the respective parties.

i2P would be a willing participant in such an arrangement at all levels – designing services, training specialist pharmacists, publicising successes and generally mentoring the service on an ongoing basis.

In the long term any professional services must evolve from their expertise in drugs, including herbals and nutritional derivatives. They must have an evidence base (from a base that is also free of corrupted input).
Most importantly they must have an infrastructure underpinning that must initially (from the contractor side) be portable and adaptable to an existing community pharmacy environment.
As an extension of that infrastructure a clinical service must be capable of being made tangible through design and its packaging into a marketing stream.
And behind all that must be a mentoring/educational system geared to training participants and delivering the content for the service.

The medical profession will run interference continually along the way because if you think about it, any extension of the utilisation of drug expertise will require the following on a progressive basis:
1. An ability to prescribe. This is already an inherent skill through existing pharmacist training. I have always resented the fact that in the early training of doctors and now clinical nurse practitioners, I have often been involved with the mentoring of these people without the legal right to prescribe myself. In a hospital environment this borders on the ridiculous, but in a clinical service environment in a community setting it simply becomes more difficult (and expensive) to complete a simple service.
Government could save more with this type of “reform” than by cutting drug prices.

2. An ability to diagnose. This has always been performed within a pharmacy environment but has not been a part of formal training. This needs to change so that pharmacists can legally diagnose to the level of their competency, without having to have a doctor “rubber stamp” the process.

3. Pharmacist clinical services should always be performed at “arms-length” so that a clinical pharmacist performing the service does not have a pecuniary interest in the pharmacy where the service was performed.
This is necessary to ensure corrupt practices do not develop and that doctors are not able to complain about not being able to own pharmacies.
The pharmacy infrastructure is a valuable component of health and this is why there are so many predators queuing up to own pharmacies (including the Colesworths)

Many critics of pharmacy clinical services might charge that clinical pharmacists are aspiring to be doctors, and if so, go and do the medical course.
Basically, this is already happening where many medical students have qualified in pharmacy before progressing to a medical school.
The type of training to be a pharmacist clinician simply raises the bar a little higher within pharmacy so that we do not lose so many graduates to the various medical schools so quickly. Pharmacist retention rate is then higher with the option still available to become a medical doctor as a form of progression, hopefully seen as part of a single strand of professional development, rather than as a competitive and destructive process along the way.
The sooner the basic philosophies are sorted out the sooner pharmacists can get on with their professional lives.
The Australian Medical Association and whatever peak body represents pharmacy (it is hard to determine these days) need to be having continuing dialogue to resolve the so called “turf issues”.
Pharmacy also needs to be able to work cohesively with all the segments representing its special interests coming together in the spirit of unity and that the realisation that they are all on the same sheet of paper making "noise" in varying degrees. That noise simply needs to become a blended choir to develop a more peaceful profession to be part of.
